Surgical Observation on the Bleeding Ulcer


Abstract
A Surgical study of surgical patients with bleeding ulcer who were admitted to the Dept. of Surgery, P.N.U.H., from Nov. 1956 to 1968, was presented.
The results obtained were as follows.
1) Of the 39 cases of bleeding ulcer, 24 cases were gastric, 14 duodenal and 1 marginal ulcer.
2) The sex incidence revealed a greater prevalence in male, with a ratio of 5.5 males to 1 female.
3) The age of the majority of the cases ranged 3rd to 4th decade(77%).
4) The seasonal distrbution was predominant in early Spring, late Autumn and early Winter.
5) Of the 39 cases, 37 were performed subtotal gastrectomy and of the rest 2 cases, one Billroth-1 type, an other resection end to end anastomosis.
6) The overall mortality rate was 7.7%. The mortality rate of subtotal gastrectomy was 8.2%, of massive hemorrhage 7.7%, of immediate operation 9.5% and of past 50 years of age 25%.
7) The postoperative results during hospitalization were as follow: among 37 cases of subtotal gastrectomy, 23(62.2%) were excellent, 11(29.7%)good, 2(5.4%) fair and 1(2.7%)poor.
8) The early postoperative complications were noted in 2 caes: one was duodenal leakage and another thrombophlebitis.
9) The each cause of deaths was shock, electrolytes imbalance and duodenal leakage.
